To enroll in Senior Health Partners, you must
- Be 55 years of age or older
- Live in Manhattan, the Bronx, Brooklyn, or Queens
- Have health problems and limitations that would qualify you for nursing home placement and require long-term care services for at least 120 days
- Be able to live safely at home with the support of Senior Health Partners
- Agree to receive all your covered services through Senior Health Partners
- Be eligible for New York State Medicaid or be willing to pay the private payment
